These are forms that we need physically signed and returned to us. We have copies of these forms at our centre for you to sign, so you can skip this section if you want. Completing these forms ahead of time will however speed up your application considerably.

 

First Aid Permission Slip - due to changes in to the Federal Government's policies for child care centre's, the centre needs all parents to complete this form and attach it to their child(ren) enrollment form.

Transportation Permission Slip - due to the nature of our centre some children may need to be transported to and from their school, this form gives permission for the centre to transport your child(ren) - this also includes excursions.

Final Acknowledgment Form - this form needs to be signed and attached to your child(ren) enrollment form.

Feedback Form - this form will help the centre inprove service and increase our standard of care.
