The forms listed below are need to be signed and returned to the centre by a legal gardian or parent of the child/ren. Completing these forms ahead of time will assist the  centre in processing all legal documents that are required to enrol your family in the centre.

First Aid Permission Slip - due to changes in to the Federal Government's policies for child care centre's, the centre needs all parents to complete this form and attach it to their child(ren) enrollment form.

Transport form - due to the nature of our centre some children may need to be transported to and from their school, this form gives permission for the centre to transport your child(ren) - this also includes excursions.

Final Acknowledgment Form - this form needs to be signed and attached to your child(ren) enrollment form.
